Geronimo have a team of passionate, proactive and well linked in Support Coordinators working with participants in Geelong and Melton.

Geronimo specialises in Level 1 and Level 2 Support Coordination and has a wealth of experience supporting children, young adults, and their families to navigate the NDIS to reach positive outcomes.

Our Support Coordinators support participants to implement their NDIS plans to maximize their supports. We assist participants to better meet their goals and link into their local community.

  • At Geronimo, Support Coordination takes a holistic approach and aims to support not just the NDIS participant, but also their family.


    Support Coordination is a service designed to help you use your NDIS plan. Support Coordinators can link you with services and therapists. They can also call you regularly to check in to see how you are going, if you need anything or what challenges you may be facing. Support Coordination is designed to build your capacity to take ownership of how and with who you engage with.

  • If you have Support Coordination funding in your plan, you will find this under Capacity Building Supports.

    If you don;t have Support Coordination funding, this is something you can request to be added into your plan at at plan review meeting with the NDIS.

  • Support Coordinators can help with each step of the plan reassessment process. We liaise with your support team to gather reports, meet with you to discuss goals and funding required in your next plan, submit documents to the NDIS, and attend the plan reassessment meeting with you.

  • Our Support Coordinators will work with you to identify and prioritize the things you need the most support with. During the intake process, we can create a plan to use your funding in a way that meets your needs.

  • Our Support Coordinators work directly with participants, parents and the participants wider support team to ensure a holistic approach and that the team are working together to best support the participant. When appropriate our Support Coordinators can meet with families all together, participants independently or a combination, developing relationships with both families and the participants.

    You will often find our Support Coordination team out for coffees with participants, visiting workplaces, meeting with other providers or hosting care team meetings.

  • Support Coordination and Plan Management are two separate roles. A Plan Manager is responsible for managing your NDIS funding and paying invoices for the services you access. A Support Coordinator’s role is to support you in linking into services, managing supports and supporting you in increasing your understaiding of the NDIS and how to use your plan.

    Some participants may have both a Plan Manager and Support Coordinator.
