The key information that is different to with these assessments are:
Assessment: There is a pre-written intro, this will just need names/dates added. This will reduce time that can be used for the analysis.
Dunn’s model is referenced within the assessment introduction.
The quadrants are explained here as well, this is to avoid double up with writing, as well as to have the definition of the quadrants.
Neurological thresholds and self-regulation: is prewritten and documented for you. Again, this is to save time with the analysis. The new Dunn’s model is also here visually.
Rationale: This is here to demonstrate clear clinical reasoning as to why the Ax was completed.
Results: Similar scoring tables as per the previous template.
Rater section comments: This section is very important, please complete this for each client. This will form your clinical reasoning. Please note down any parent/carer/participant comments here.
Sensory Review: This has changed from ‘quadrant review’. This is to enable more flexibility and to focus on the thresholds, and active/passive regulation. You can refer to the quadrants here, but keep the focus on the processing style/pattern (i.e., threshold and regulation type). Please watch the group supervision video to obtain more information on this. Here, we want to really focus on the individual. Recommendations will be aligned with what is written in the thresholds, this will make it easier to match what is recommended, as it will be more individualised rather than quadrant focused.
Recommendations at the end are for the NDIS recommendations, or more core recommendations for items.
